Com esta nova mudança a farmácia passou a interagir mais com a comunidade, sendo esta um espaço dedicado à saúde do utente. O contributo do farmacêutico assume um papel fundamental e são estabelecidas relações de proximidade, que assentam no aconselhamento, no ensino, no tratamento que foi instituído e na criação de estratégias que garantam a sua efetividade. O desafio ainda se torna mais elevado quando se aborda o doente crónico, uma vez que a interação se mantem constante ao longo da vida do utente. Aborda-se a Diabetes nesta temática, pois é uma patologia com a qual o farmacêutico comunitário consegue ter abordagem contínua ao longo da doença, e é aqui que este se diferencia dos outros profissionais, pois não se limita apenas à dispensa do medicamento. A adoção por parte do farmacêutico de intervenções múltiplas e multidisciplinares, que respeitem a singularidade do doente, permite um maior controlo de problemas de saúde. A farmácia da atualidade acompanhou o avanço das novas tecnologias, permitindo uma maior proximidade e acessibilidade a todas as faixas etárias. Estas são também utilizadas para um melhor controlo e uma melhoria da qualidade de vida dos doentes.Nowadays, the role of a Pharmacist has a great importance on the evolution of a pharmacy. With this new mindset the pharmacy becomes to interact more with the communities and a new dedicate healthcare space arises to the patients. The contribution of a Pharmacist is fundamental to establish deep relations, where support and education on any treatment are key to get the desire success. The challenge is more serious when we have a chronic patient, as his support should be maintained through is entire future life. In this work we deal with Diabetes as it is a pathology which a Pharmacist has a continuous relation with the patient all over the disease and is here, he becomes different from the other healthcare professionals, because his work will not finish when sell the medicine. Actually the Pharmacist adopt several and multidiscipline interventions which permits (him) to have a better control on healthcare problems. Today’s pharmacy follows the advance of new technologies, giving a better proximity and accessibility to all generations.
