Padrões de qualidade em unidades de cuidados continuados integrados: perceção dos fisioterapeutas

A Rede Nacional de Cuidados Integrados (RNCCI) tem como missão “Prestar os cuidados adequados, de Saúde e apoio social, a todas as pessoas que, independentemente da sua idade, se encontrem em situação de dependência.” Paralelamente, assiste-se a uma crescente preocupação com os padrões de Qualidade nos serviços de saúde, onde se incluem as Unidades de Cuidados Continuados Integrados (UCCI). O âmbito de intervenção na Rede fundamenta-se em três princípios fundamentais: reabilitação, readaptação e reinserção.
Neste contexto planeou-se um estudo com a participação dos Fisioterapeutas, considerando que são profissionais, por excelência, diretamente ligados à reabilitação e consequentemente à readaptação e reinserção, assumindo por isso um papel de grande relevo na RNCCI. Sabendo que a sua intervenção tem como objetivo atingir e manter um nível de funcionalidade adequado a cada indivíduo e minimizar a percentagem de dependência através de um plano de intervenção planeado com o utente, realizou-se um estudo descritivo, pois consiste numa investigação aprofundada de um grupo de indivíduos, num número limitado de instituições. Em termos metodológicos, foi utilizado um questionário auto preenchido, construído com base na “Grelha de Avaliação da Qualidade das Unidades da Rede Nacional de Cuidados Continuados Integrados”. A população em estudo foi uma amostra de conveniência, aleatória, composta por 20 fisioterapeutas, aos quais foi aplicado o questionário.
O objetivo do estudo foi proporcionar informação sobre a perceção dos Fisioterapeutas em funções em instituições da Rede Nacional de Cuidados Continuados, relativamente aos padrões de Qualidade praticados. A escolha das unidades de saúde foi feita por conveniência e mediante autorização das respetivas direções.
Concluiu-se que a “Grelha de Avaliação da Qualidade das Unidades da Rede Nacional de Cuidados Continuados Integrados” mede de forma aceitável, nesta amostra, os dois componentes em estudo: “Prestação de Cuidados de Reabilitação” e “Gestão do utente na Unidade”, pelo que podemos utilizar os itens da grelha para constituir variáveis representativas dos dois componentes.Ambos são bastantes valorizados pelos fisioterapeutas e não se verificam diferenças estatisticamente significativas entre a valorização que os fisioterapeutas fazem de cada um dos componentes. A perceção dos fisioterapeutas sobre os padrões de qualidade, determinada pelo componente “Gestão do utente na Unidade” explica 34,5% da valorização da prestação diária de cuidados de saúde e reabilitação, determinada pelo parâmetro “Prestação de Cuidados de Reabilitação”, de uma forma estatisticamente significativa.
The National Network of Integrated Long-term Care has as mission “To provide proper care, health and social support to all people, regardless of age, who are in a situation of dependency.” Parallel, an increasing concern with the standards of Quality in the health services is attended to it, where are included the Units of Integrated Long-term Care. The scope of intervention in the Network is based on three basic principles: rehabilitation, readjustment and reintegration. In this context, a study with the participation of the Physiotherapists was planned, considering that they are the healthcare professional spreferably positioned to deal directly with rehabilitation, and the readjustment plans together with reintegration for an upper excellence level of continuous care achievements, assuming therefore a role of great relief within the Network. Knowing that its intervention has as objective to reach and to keep a level of adequate functionality to each individual as well as to minimize the percentage of dependence through a gliding plan of intervention with the patient, a descriptive study was performed, designed as acase study, therefore consisting of a deepened inquiry of a group of individuals, in an limited number of institutions. In methodological terms, an auto questionnaire was filled, constructed de novoon the basis of the “Grid of Evaluation of the Quality of the Units of the National Network of Integrated Long-term Care”. The population in study was a randomized sample of convenience, composed by20 physiotherapists, whom the questionnaire was applied. The goal of the study is to provide information on the perception of the Physiotherapists in functions in institutions of the National Network of Long-term care, relatively to the practiced standards of Quality. The choice of the units of health was made by convenience and by means of authorization of the respective directions. The main conclusions are that the “Grid of Evaluation of the Quality of the Units of the National Network of Integrated Long-term Care” may enable acceptable measurements, as applied in this sample, for the two parameters in study: “Rehabilitation Care Provision” and “Organization and Functioning of the Unit.” Hence, the items of the Gridmay be used to constitute representative variables of the two parameters. Both parameters, “Rehabilitation Care Provision” and “Organization and Functioning of the Unit” are sufficient valued by the physiotherapists and no statistic differences are detected between the independent valuation that the physiotherapists make of each parameter. The perception of physiotherapists on the Quality standards as determined by the component "Organization and Functioning of the Unit" explains a statistically significant 34.5% value of daily delivery of health care and rehabilitation programs, determined by the parameter “Rehabilitation Provision”.
