As bradiarritmias e os bloqueios de condução cardíaca poderão ser hemodinamicamente significativos e requerer um tratamento médico ou cirúrgico. A implantação de pacemakers é o tratamento de eleição destas desordens de ritmo em cães sintomáticos, nomeadamente em bloqueios atrioventriculares de grau avançado e de terceiro grau, doença do nódulo sinusal, standstill atrial persistente e síncope neurocardiogénica. Este procedimento é minimamente invasivo e apresenta uma elevada taxa de sucesso na redução dos sinais clínicos e no aumento do tempo de sobrevivência.
Neste estudo retrospetivo caracterizou-se a população de 13 cães submetidos a colocação de pacemaker cardíaco artificial, no Hospital Veterinário do Porto, entre abril de 2015 e maio de 2021. Objetivou-se analisar as indicações clínicas, a presença de complicações e o sucesso terapêutico.
A síncope constituiu a manifestação clínica mais frequente. O bloqueio atrioventricular de terceiro grau foi o diagnóstico eletrocardiográfico mais comum. Ocorreram 10 complicações: 5 maiores e 5 menores. Até à data de conclusão do estudo, faleceram seis cães. Desses, apenas um óbito teve relação com a implantação do dispositivo. O tempo médio de sobrevivência para todos os cães após o procedimento foi de 1096 dias. Ao fim de 6 meses, cerca de 85% dos cães estavam vivos.
Bradyarrhythmias and cardiac conduction disturbances might be haemodinamically significant and require medical or surgical treatment. Implantation of pacemakers is the treatment of choice for these rhythm disorders in symptomatic dogs, such as in high grade and third grade atrioventricular blocks, sick sinus syndrome, persistent atrial standstill and neurocardiogenic syncope. This procedure is minimally invasive and is associated with a high success rate in reducing the clinical signs and enhancing life expectancy. This retrospective study aims to characterise the population of 13 dogs who received artificial cardiac pacemaker implantation in Oporto Veterinary Hospital since april 2015 until may 2021. The indications for pacing, complications and therapeutic success were also examined. Syncope was the most reported clinical sign. Third grade atrioventricular block was the most common indication for the surgical procedure. There were 10 complications, of which 50% were minor and 50% were major. Six dogs were dead by the end of the study. Only one death was atributed to the pacemaker implantation. The mean survival time for all dogs after the procedure was 1096 days.
