Plano individual de intervenção: proposta para uniformização nas Unidades de Internamento da Rede Nacional de Cuidados Continuados Integrados

Problema de investigação: A RNCCI não desenvolveu uma imagem e linguagem comum para o Plano Individual de Intervenção o que poderá prejudicar a eficácia da comunicação dentro da própria Rede, nomeadamente, quando existe transferência de utentes entre os vários níveis de cuidados. Para além disso, apesar de existirem indicadores de Ganhos em Saúde na RNCCI, estes não quantificam os ganhos em funcionalidade que sustentam a filosofia da Rede e não são comparáveis internacionalmente.
Questão orientadora: Como incluir o modelo conceptual da Classificação Internacional de Funcionalidade, Incapacidade e Saúde no Plano Individual de Intervenção das Unidades de internamento da Rede Nacional de Cuidados Continuados Integrados? Objectivo geral: Contribuir para o desenho do Plano Individual de Intervenção das Unidade de Internamento da Rede Nacional de Cuidados Continuados Integrados.
Objectivos específicos: Analisar e caracterizar o Plano Individual de Intervenção em uso nas Unidades de Internamento da Rede Nacional de Cuidados Continuados Integrados; Definir um Core Set da Classificação Internacional de Funcionalidade, Incapacidade e Saúde para os utentes da Rede Nacional de Cuidados Continuados Integrados.
Tipo de estudo: Utilizou-se a metodologia qualitativa, num estudo descritivo, com diferentes métodos de recolha de dados, análise de conteúdo e método de Delphi modificado – revisão sistemática de literatura e painel de Delphi.
Conclusões: A análise efectuada aos Planos Individuais de Intervenção em uso por Unidades da RNCCI permitiu constatar as grandes diferenças de apresentação, de forma de registo e de objectivos da utilização que existem relativamente aos PII’s. Através da Revisão Sistemática de Literatura e do Painel de Delphi definiram-se 27 códigos da Classificação Internacional de Funcionalidade e Saúde que poderão caracterizar os utentes da RNCCI. Foi proposto um modelo para o Plano Individual de Intervenção que integra o modelo conceptual da Classificação Internacional de Funcionalidade e Saúde que deverá ser validado posteriormente.

Furthermore, even though there are indicators of health gains in RNCCI, they do not quantify the gains in functionality that support the philosophy of the Network and are not internationally comparable. Guiding question: How to include the conceptual model of the International Classification of Functioning, Disability and Health in Individual Intervention Plan inpatient Units of the National Network for Continuing Care? General objective: To contribute to the design of the Individual Intervention Plan Inpatient Unit of the National Network of Integrated Care. Specific objectives: To analyze and characterize the Individual Intervention Plan in use in inpatient Units of the National Network of Continuous Care; Defining a Core Set of International Classification of Functioning, Disability and Health for users of the National Network for Integrated Continuous Care. Study design: It was used a qualitative methodology in a descriptive study with different data collection methods, content analysis and modified Delphi method - a systematic literature review and Delphi panel. Conclusions: The content analysis of Individual Intervention Plans in use by RNCCI units have revealed large differences in presentation, registration and use objectives that exist in relation to PII 's. Through Systematic Literature Review and Delphi panel set up 27 codes from the International Classification of Functioning and Health that may characterize RNCCI users.
