Prevalência da doença hemolítica perinatal e seus determinantes: variação com a posição sócio-económica Açores, São Miguel

Abstract

A Doença Hemolítica Perinatal ocorre devido à passagem de anticorpos, da circulação materna para a fetal, através da placenta, o que origina a hemólise dos eritrócitos fetais e, em casos extremos, a morte. No presente estudo faz-se uma abordagem inovadora na área, para melhor compreender a presença da DHP, em S.Miguel. Aplicou-se o método retrospetivo, documental e transversal, entrevistando 118 gestantes que foram seguidas no HDES, nas consultas de obstetrícia, no serviço de Hematologia. O tratamento dos dados realizou-se estatisticamente pelo SPSS®, o que permitiu efectuar análises quantitativas e de correlação. Os dados obtidos revelaram que o concelho de Ponta Delgada e a Vila de Rabo de Peixe possuem mais casos, sendo que, ao longo do tempo, os mesmos se têm verificado estáveis. As gestantes apresentam uma média de 29 anos; revelam baixa literacia; são domésticas; têm baixo rendimento; vivem com o marido em coabitação com familiares, sendo o agregado composto por 3 pessoas por habitação. Das gestantes empregadas, aproximadamente, metade, trabalha por conta de outrem. Em termos clínicos, são predominantemente A negativas; multíparas e com baixa percentagem de abortos. Não foi possível verificar a correlação entre variáveis ,porque a correlação de Sperman se revelou insignificativa.

The Hemolytic Perinatal Disease occurs due to the passage of antibodies from the mother to the fetal circulation through the placenta, reacting and resulting in hemolysis of fetal red blood cells, leading cases, death. In this study we make an innovative approach in the area, aiming to better understand their presence of DHP, in S.Miguel. The method applied was retrospective, documental and cross checked, based on a sample of 118 pregnant women who were followed in the HDES, in the high-risk medical check in the Hematology service. Data analyses were performed statistically with the SPSS, allowing the quantitative and the correlation analysis. The results revealed that the municipality of Ponta Delgada and the borough of Vila de Rabo de Peixe have more cases and over time the number of case have been found stable. Pregnant women are on average 29 years old, have a low literacy, are dedicated to the domestic work, have a low income, live with her husband in cohabitation with family (on average of 3 people). Of the working pregnant women, near half of them are employed. In clinical terms, they are predominately A negative, multiparous and with a low percentage of abortions. It was not possible to verify the correlation between variables because the correlation of Spearman proved insignificant.
