Associação entre a experiência negativa do parto e trauma perinatal : o papel moderador de fatores sociodemográficos e clínicos

A Perturbação de Stress Pós Traumático Perinatal (PPST-P) afeta a saúde física e emocional da mãe, a relação conjugal e os cuidados ao bebé, sendo crucial explorar os seus fatores de risco. O presente estudo teve como objetivo explorar a associação entre a experiência negativa do parto e sintomatologia de PPST-P e compreender se essa associação é moderada por história prévia de psicopatologia, experiências traumáticas na infância (abuso sexual e maus-tratos físicos e emocionais), presença de companheiro e rendimento familiar. Um total de 550 mães preencheram um conjunto de questionários por via online referentes à experiência de parto, história da psicopatologia prévia, história de maus-tratos na infância e sintomatologia de PSPT. Os resultados demonstraram que experiências negativas no parto estão associadas ao aumento de sintomatologia de PPST- P e esta associação é mais forte para mulheres com história de maus-tratos (físicos, psicológicos/emocionais e/ou abusos sexuais na infância), história prévia de psicopatologia (ansiedade e depressão) e sem companheiro. O rendimento familiar não moderou a associação entre experiência negativa do parto e sintomatologia de PPST-P. A compreensão desses fatores de risco específicos é crucial na prevenção e intervenção eficaz na PPST-P, promovendo melhores cuidados de saúde mental no período pós-parto.
Perinatal Post Traumatic Stress Disorder (P-PTSD) affects the mother's physical and emotional health, the marital relationship and the care of the baby, and it is therefore crucial to explore its risk factors. The aim of this study was to explore the association between the negative experience of childbirth and P-PTSD symptoms and whether this association is moderated by a previous history of psychopathology, traumatic experiences in childhood (sexual abuse and physical and emotional abuse), the presence of a partner and family income. A total of 550 mothers completed a set of online questionnaires regarding their childbirth experience, history of previous psychopathology, history of childhood maltreatment and P-PTSD symptoms. The results showed that negative childbirth experiences are associated with increased P-PTSD symptoms and this association is stronger for women with a history of abuse (physical, psychological/emotional and/or sexual abuse in childhood), a previous history of psychopathology (anxiety and depression) and no partner. Family income did not moderate the association between negative childbirth experience and P-PTSD symptoms. Understanding these specific risk factors is crucial in preventing and effectively intervening in P-PTSD, promoting better mental health care in the postpartum period.
