A trombose é definida como a formação de um trombo num vaso ou câmara
cardíaca, e pode resultar na obstrução parcial ou completa do vaso de origem ou de
um desprendimento do trombo e consequentemente um tromboembolismo num local
distante. No cão as desordens tromboembólicas estão associadas a doenças comuns
em clínica de pequenos animais como coagulação intravascular disseminada, anemia
hemolítica imunomediada, doença cardíaca, sepsis, neoplasia, hiperadrenocorticismo,
nefropatia com perda de proteína, pancreatite aguda necrosante e diabetes mellitus.
O estudo retrospectivo desenvolvido no presente trabalho incluiu uma
amostra de 10 canídeos com trombos alojados em vasos arteriais 5 (50%) e em vasos
venosos 5 (50%). A veia cava 3 (23%) e a artéria aorta 3 (23%) foram os locais de
deposição com maior prevalência, seguidos da veia porta 2 (15%), artéria pulmonar 2
(15%), veia frénica 1 (8%) e veia braquiocefálica 1 (8%). A doença concomitante mais
prevalente foi neoplasia ou suspeita de neoplasia (massas sólidas) 7 (70%), seguido
de trauma 1 (10%), enteropatia com perda de proteína 1 (10%) e um dos casos não
tinha doença concomitante 1 (10%).
A tomografia computorizada com angiografia provou ser um exame
complementar de diagnóstico útil na detecção de trombos. Dada a raridade da
prevalência descrita na bibliografia de trombos alojados na artéria aorta e na veia
cava, podemos colocar a hipótese que a doença tromboembólica é subdiagnosticada
em medicina veterinária.
Thrombosis is defined as the formation of a thrombus within a blood vessel or a cardiac chamber, it may result in the partial or complete obstruction of the primary vessel or the thrombus dislodging and consequently a thromboembolism occurs in a distant vessel. Canine thrombotic disorders are associated to common diseases seen in small animal practice such as disseminated intravascular coagulation, immune mediated hemolytic anemia, cardiac disease, sepsis, neoplasia, hyperadrenocorticism, protein losing nephropathy, acute necrotizing pancreatitis and diabetes mellitus. The retrospective study in this work included a sample of 10 canines with lodged thrombus in arterial vessels 5 (50%) and in venous vessels 5 (5%). The vena cava 3 (23%) and the aorta 3 (23%) were the most prevalent locations of lodging, followed by the portal vein 2 (15%), pulmonary artery 2 (15%), phrenicoabdominal vein 1 (8%) and the brachiocephalic vein 1 (8%). The most common concurrent disease was neoplasia or suspected neoplasia (solid masses) 7 (70%), followed by trauma 1 (10%), protein losing enteropathy 1 (10%) and one of the patients had no concurrent disease 1 (10%). Computerized tomography angiography demonstrated to be a useful complementary diagnostics exam in the detection of thrombus. Given how rare the prevalence described in the bibliography of lodged thrombus in the vena cava and aorta, we can relate the hypothesis that the thromboembolic disease is underdiagnosed in veterinary medicine.
