Esta dissertação elaborada como trabalho final de conclusão do Mestrado Integrado de Medicina Veterinária, na Universidade Lusófona - Centro Universitário Lisboa, teve como objetivo aprofundar o tema das opções cirúrgicas para correção de luxação de patela em animais de companhia. O presente relatório inclui uma vasta revisão bibliográfica para dar suporte aos 4 casos clínicos de cães com patologia de luxação de patela, que foram selecionados durante o estágio curricular no Anicura Restelo Hospital Veterinário, em Lisboa, e no Bonematrix - Centro de Cirurgia Veterinária Avançada, Porto, ambos em Portugal. A luxação de patela é uma patologia ortopédica específica, cada vez mais diagnosticada em cães, afeta os membros pélvicos e pode ser classificada em luxação medial ou lateral e ainda quanto ao grau de luxação que apresenta, com um sistema de classificação de 4 graus. Esta patologia pode ser abordada com um tratamento conservador, caso não apresente um grau de luxação elevado e não tenha sinais clínicos associados. Relativamente à abordagem cirúrgica desta patologia, devem ser tidas em conta as várias alterações que conduzem à luxação de patela, sendo por isso fundamental um estudo aprofundado do membro pélvico afetado, de forma a chegar a um diagnóstico final para elaborar um correto planeamento cirúrgico, que pode ser simples ou englobar uma combinação de várias técnicas. Apesar de ser fundamental um planeamento bem realizado, existem circunstâncias em que algumas tomadas de decisão só podem ocorrer durante o momento cirúrgico. Dentro das várias técnicas cirúrgicas para correção da causa primária, descritas neste trabalho, fazem parte osteotomia corretiva de fémur distal, osteotomia corretiva de tíbia proximal, transposição da tuberosidade tibial, patellar groove replacement, trocleoplastia, desmotomia medial/lateral, capsulorrafia lateral.
This dissertation, prepared as the final project for the integrated master’s degree in veterinary medicine at Lusófona University – Lisbon University Centre, aimed to explore the topic of surgical options for correcting patellar luxation in companion animals. This report includes an extensive literature review to support the four clinical cases of dogs with patellar luxation pathology, which were selected during the curricular internship at Anicura Restelo Veterinary Hospital in Lisbon and Bonematrix - Advanced Veterinary Surgery Centre in Porto, both in Portugal. Patellar luxation is a specific orthopaedic pathology that is becoming increasingly common in dogs. It affects the pelvic limbs and can be classified as medial or lateral luxation, and also according to the degree of luxation, with a 4-degree classification system. This pathology can be treated conservatively if the degree of dislocation is not severe and there are no associated clinical signs. Turning to the indication for surgery, several primary causes lead to patellar dislocation. It is therefore essential to conduct a full examination of the affected pelvic limb in order to reach a final diagnosis and develop a correct surgical plan, which can be simple or involve a combination of techniques. Although it is essential to plan well, there are circumstances in which some decisions can only be made during surgery. The surgical techniques for correcting the primary cause described in this dissertation include corrective osteotomy of the distal femur, corrective osteotomy of the proximal tibia, transposition of the tibial tuberosity, patellar groove replacement, trochleoplasty, medial/lateral desmotomy and lateral capsulorrhaphy.
